Community Health Improvement Workgroup
Mission: To improve the health and wellness of individuals and families of Monroe County by addressing prioritized needs and inequities through sustainable systems change built on collaboration and supported by shared resources.
The Community Health Improvement Workgroup of Monroe County provides remarkable examples of how leaders from hospitals and the community can collaborate to improve the health of the population.
There are two primary hospital systems in the region, each operating two hospitals in Monroe County. Every three years, through a process mandated by the Affordable Care Act and the New York State Department of Health, non-profit hospitals, and the health department conduct a Community Health Needs Assessment (CHNA) to determine areas of community health concern. In addition, a Community Health Improvement Plan (CHIP) is developed every three years to address the health needs of the County.
In Monroe County, the Community Health Improvement Workgroup (CHIW) meets monthly, and brings together leaders from hospitals, health departments, and community agencies to prioritize community health needs. The CHIW has been meeting monthly since 2000.
The CHIW offers a Mini-Grant program that awards and supports community-based agencies working in tandem with the goals of the CHIP. The CHIW partners with mini-grant awardees, and other community-based organizations, to work collaboratively for the mutual benefit of all groups, and ultimately for the benefit of the residents of Monroe County.

Upcoming Meetings
The CHIW typically meets on the third Monday of every month at 3:30 p.m. Meetings are currently being held remotely.
