Referrals and Billing

The prospective patient or family member should contact the health insurer to determine if a referral is necessary. If a referral is necessary, the patient or family should contact the primary care physician to obtain a referral for the specific physician or nurse practitioner the patient will see. A portion of charges for the visit are covered by most major health insurers. The patient will be responsible for co-payment or fees that are not covered by insurance. Memory Care Program participates with Blue Choice, MVP Healthcare, Medicare, Medicaid, and Excellus Blue Cross Blue Shield of Rochester health plans.
