The New York State Prevention Agenda is a five-year plan and dashboards of indicators are available through the Department of Health and the Prevention Agenda for the state and county-level data.
Monroe County’s Department of Public Health publishes reports on the Youth Risk Behavior Survey, both at a County Level and for the Rochester City School District. The department collects, analyzes and shares data about chronic disease, adult health, community trends and acute issues like the opioid crisis. Those reports can be found here.
Common Ground Health brings focus to community health issues via data analysis, community engagement and solution implementation through community collaboration and partnership. County Health Ranking and Roadmaps is a compilation of data and community health improvement tools in collaboration with the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute. Monroe County's rankings can be found here.
The Metro Council for Teen Potential (MCTP) is a coalition that unites the community behind proven best practices so that youth reach their full potential. MCTP promotes strength-based, evidence-based and coordinated strategies to foster youth development and encourage young people to make healthy choices and avoid unplanned pregnancy.” Their 2017 Needs Assessment was used as a reference in our County-wide Community Health Needs Assessment.

The Rochester RHIO(Regional Health Information Organization) is a secure, electronic health information exchange serving authorized medical providers and over 1.4 million patients in Monroe, Allegany, Chemung, Genesee, Livingston, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming and Yates counties in upstate New York.
