Partnering & Grants

The Center for Community Health & Prevention partners with the community to help residents of Rochester and the Finger Lakes region lead healthier lives.

There are multiple opportunities to partner with us on health-related initiatives. Grants, Community Engagement Studios and other research initiatives are a few of the ways we can work together.

  • Community Health Mini-GrantsSupported and funded by the Center and the UR Clinical & Translational Science Institute, the Mini-Grants are one-time grants up to $2,000. Up to two applicants will be selected during the application period. The funding period is July 1, 2025 through May 31, 2026. Applicants can apply to one community project each year per partnership.
  • Community Health Improvement Workgroup (CHIW) Mini-Grant: The CHIW accepts proposals annually to fund those who have expertise in either of the three priority areas highlighted in the 2025-2027 Monroe County Joint Community Health Improvement Plan (CHIP). Those three priority areas are 1) Maternal Health, 2) Anxiety and Stress, and 3) Poverty.
