Carter Catlett Williams

Introduction

Carter Catlett WilliamsCarter Catlett Williams attained national prominence in the 1980s and 1990s as a geriatric social worker and advocate of nursing home reform. She was particularly active in the movement to abolish the use of physical and chemical restraints in nursing home settings. The bulk of the collection dates from the final two decades of the 20th century – years during which Carter Williams was most active as a consultant, speaker and author.

Ms. Williams was active in regional, state and national organizations focused on long-term care and nursing home reform – activity well represented in the collection. Many of the files in this collection also represent her attendance at conferences at which she delivered talks (e.g., the American Society on Aging, the Gerontological Society of America, etc.) or at sponsored events to which she had been invited as guest speaker. Nearly all these files contain copies of her talks or detailed notes upon which her remarks were based. The papers of Carter Catlett Williams were presented to the Edward G. Miner Library in March 2013. Processing the collection was completed in July 2014. The collection is contained in 19 boxes occupying eleven linear feet.

Biography

Catharine Mott Catlett (Carter Catlett Williams) was born in Texas on September 2, 1923, the daughter of Landon Carter Catlett, Jr., (1898-1925), a 2nd lieutenant in the U.S. Army Air Service, and Catharine Mott Catlett (1900-1986). Both were natives of Gloucester County in the Tidewater region of Virginia. After the death of her husband in an air crash in Hawaii, Carter Williams’ mother returned with her two year old daughter to her parents’ home in Gloucester County. A lovely reminiscence of growing up at this period and in this environment is provided in the introductory chapter to Glorious adventure, a collection of her father’s letters compiled and narrated by Carter Williams that was published in 2008 (Box 17, Folder 4).

It was at this time that Catharine’s mother changed her daughter’s middle name from Mott to Carter. Some years later, finding herself one among several Catharines at St. Catharine’s School (Richmond) and in her class at Wellesley, Catharine adopted Carter as her first name. Carter Catlett graduated from Wellesley College in 1945. She returned home to work in the Gloucester County social welfare department before entering the Simmons School of Social Work (Boston), from which she received her masters degree in 1949. While in graduate school, she met Thomas Franklin Williams, a native of North Carolina and a student at Harvard Medical School. They were married in 1951 and had three children: Mary Wright (b. 1956), Thomas Nelson (b. 1959), and Landon Carter, who died prematurely as an infant on April 19, 1961.

When T. Franklin Williams, M.D. joined the medical faculty at the University of North Carolina, Carter Williams maintained her interest in social work while raising two small children. In 1968 the Williams family moved to Rochester, N.Y., where Dr. Williams had been appointed medical director at Monroe Community Hospital and professor at the University of Rochester School of Medicine and Dentistry.

As her children grew, Carter Williams returned part-time to social work. Her increasing concern for the emotional and physical condition of elders in nursing homes paralleled in part her husband’s specialization in geriatric medicine.

In 1974 Carter Williams began to work independently as a nursing home consultant. Most nursing homes established in the United States during the 1960s operated on the model of acute care hospitals – a model that Williams sensed was less concerned about the personal needs of elderly patients than in conforming to the protocols of a system. She was particularly concerned about the widespread use of physical and chemical restraints in the care of elderly residents. During the 1980s and 1990s, Williams argued that simple changes in the physical environment and in attitudes toward patient care in nursing homes would make the use of restraints unnecessary. A Commonwealth Fund grant-funded tour of nursing homes in Scotland and Sweden – particularly at Grabergets, Sweden – provided Williams successful practical models to argue for the elimination of restraints in the United States (see Box 2, Folders 7-12, Box 3, Folders 1-4).

In 1983, Carter and T. Franklin Williams moved to Bethesda, Md. when Dr. Williams was appointed director of the National Institute on Aging of the National Institutes of Health. Carter Williams was associated on a part-time basis with the social work department of the Hebrew Home of Greater Washington and continued her work as an advocate for nursing home reform. She served as a consultant to the group developing criteria for nursing home assessment following the Omnibus Reconciliation Act (OBRA) of 1987 (see Box 5, Folders 8-13, Box 6, Folders 1-4); was active with the Foundation for Long Term Care (Box 3, Folders 13-14, Box 4, Folders 1-8), the National Citizens’ Coalition for Nursing Home Reform (Box 9, Folders 18-24, Box 10, Folders 1-4), and the National Restraint Minimization Project (Box 10, Folders 9-13, Box 11, Folders 1-3). Throughout the 1980s and 1990s, Carter Williams lectured coast to coast on the need to eliminate physical and chemical restraints and published widely on this topic.

In 1991 the Williams returned to Rochester. Carter Williams continued in her role as consultant, author and lecturer while her husband assumed new responsibilities with the Monroe Community Hospital, the Veterans Administration, and the University of Rochester School of Medicine & Dentistry.
