Primary Care Practice Transformation Fellowship
About Finger Lakes Center for Primary Care Clinician Education
Based at the University of Rochester Medical Center Department of Family Medicine, the FLC-PCCE will work with primary care practices across all health systems, to:
- Collaboratively assist primary care practices in the 14-county Finger Lakes Region to transform practices to thrive in an integrated, value-based healthcare delivery system
- Engage with Primary Care clinicians, (including MD, DO, PA, NP) to provide high quality, evidence based teaching so participants learn and can implement strategies to change practice
About The Fellowship
- A one-year, part time fellowship, over 9 months
- Trains Primary Care Physicians, Nurse Practitioners, Physician Assistants, and their office staff
- Shares practical skills and strategies to transform practices to thrive in a value-based reimbursement system
- Learning collaborative format
- Participants will gather with a facilitator to review topics and share innovations in their practices
- Brief didactic with skills practice, shared problem solving
- Participant mini-projects between sessions to develop and carry out rapid cycle tests of change relevant to their own practices
- Stipend for clinician participants up to $10,000 to offset expenses associated with time away from their practice.
- Stipend for staff member participation to cover time away from the practice.
