Children and Families

The Center for Community Health & Prevention's clinical team provides family-centered care. Recognizing the critical role family dynamics and social determinants of health play in health outcomes, our clinical team offers services focused on family-centered health promotion and chronic disease prevention, involving each family member in the process. 

We provide evidence-based guidance and support from a family perspective, as well as counseling patients and families to increase awareness and knowledge of healthy practices and self motivation to make healthy choices.
