Golisano Children's Hospital is developing broad partnerships with community organizations, foundations, and providers to address the specific, local needs of our children.
Why It’s Important. And How We Will Do It
Ensuring our next generation is healthy and well is not only the right thing to do, but the smart thing to do. Focusing on people’s health in the younger, formative years will positively impact their personal development and societal contributions. In Rochester, our community pediatric clinicians and community based organizations are organizing resources to focus on best outcomes for infants and toddlers.
Goals & Accomplishments
Goal: Implement Universal Newborn Telehome Visitation
Accomplishments:
- Through ROC Family TeleConnects program, 1,500 newborns and families have received health/telehealth visits in the city of Rochester (birth cohort 3000)
- Secured 2.7 million in funding from NYS and Excellus to date for telehealth
- Excellus has now implemented this in their standard funded portfolio for MMC and commercial lines
- Platform funded and developed to support county-wide data collection and reporting
Goal: GROW Comprehensive Developmental Screening
Accomplishments:
- 7112 children (majority 2, 3 and 4 year-olds in city of Rochester) screened and over 50% identified with delays were supported for needed referrals in last 4 years (birth cohort: 12,000)
- Funding to date: $4.9M from Medicaid, NYS, AHP and MVP
- Expanding to 5 additional counties in the Finger Lakes region
Goal: Advocate for Medicaid Policy Change
Accomplishments:
- NYS First 1000 Days Initiative for maternal-child health implemented across state; including Monroe and Chemung counties
- Commitment to pursue maternal child health and mental/behavioral health in Finger Lakes region in NYS Medicaid Waiver
Goal: Implementation of Parent/Family Advocacy Partner Leadership, and Integration of Health, Education and Human Services
Accomplishments:
- Launch of Whole Child Initiative within ROC the Future Alliance based on All Kids Thrive Model
- Funding to date: $1.7M committed funding per year for 3 years from local foundations prioritizing whole child health
